Qualifying member may receive a monthly pension payment based on their plan’s formula, even if they are not yet eligible for retirement benefits. Each plan’s criteria and formula for disability benefits is different.
To Qualify for Disability
Your disability must have lasted or be expected to last at least one year to qualify for disability benefits from PERA. Each plan has specific requirements that must be met for a disability benefit. Learn more about disability benefits in the disability information publications and also your member handbooks.
You can request a disability packet to start the disability application process. The disability packet includes disability information, estimates, and application forms. You can request a disability packet through your myPERA account or by calling our Member Service Center.
Application Documents
Complete the following disability paperwork to apply for disability benefits:
Notarized Disability Application
Proof of your date of birth and any name changes
Two completed medical reports
Federal and MN (if applicable) tax withholding forms
If electing a survivor benefit, proof of survivor's date of birth and any name changes
When Benefits Begin
Disability benefits begin when you no longer have sick or vacation time credited to you and are no longer receiving any other salary payments. Benefits can be paid retroactively, but no more than 90 days prior to the day PERA receives your application.
